Madrigal is a biopharmaceutical company focused on delivering novel therapeutics for metabolic dysfunction‑associated steatohepatitis (MASH), a serious liver disease that can progress to cirrhosis, liver failure, need for liver transplantation, and premature mortality. Our medication, Rezdiffra, is a once‑daily, oral, liver‑directed THR‑β agonist approved by both the FDA and European Commission for treatment of MASH with moderate to advanced fibrosis (F2–F3). An ongoing Phase 3 outcome trial is evaluating Rezdiffra for compensated MASH cirrhosis (F4c).

Location: Waltham, MA (Hybrid).

Summary

This individual will report to the Head of Patient Access Operations and provide operational leadership and oversight of the Access & Reimbursement Case Management (CM) team within Madrigal Patient Support (MPS). The Director will be responsible for executing a refined case management program, overseeing day‑to‑day activity of the case managers, and ensuring program performance.

Responsibilities

Operate and oversee a case manager team that conducts patient, HCP, payer, and SP outreach, and provides access, reimbursement, and affordability support to patients prescribed a Madrigal drug enrolled in MPS.

Develop and implement KPIs and oversight mechanisms to ensure ongoing execution of program support and that operations meet Madrigal’s performance and quality expectations; prepare and provide regular updates and business reviews for leadership and relevant cross‑functional teams.

Partner with MPS vendors to create program materials, call guides, work instructions, and resources to direct operations of access, reimbursement, and affordability support for patients.

Develop and ensure alignment and compliance of standard operating procedures across MPS offerings; support preparation and execution of routine program audits.

Establish strong relationships and effective communication with key internal stakeholders and outside vendors to support development of program support offerings/materials and enable continuous improvement of patient access and experience.

Provide education and information to internal teams, HCPs, and patients, using materials and messaging approved for the audience, to create awareness of program offerings and support services.

Qualifications

Minimum of a bachelor’s degree in a related field or science; MBA or advanced degree preferred.

Five or more years of pharma/biotech experience in Market Access, Patient Support/Services, or Distribution, with hepatology/gastroenterology experience preferred.

Strong knowledge of health insurance, including managed care products, state and government‑provided benefits, and specialty medication distribution channels.

Prior supervisory or management experience required, preferably in the patient services space.

Experience with large specialty drug preferred, but not required.

Demonstrated experience adapting to change, navigating ambiguity, and executing with autonomy.

A self‑starter with a continuous‑improvement mindset who drives progress, execution/delivery, and performance of initiatives/programs.

Passion for helping patients navigate their medication and/or healthcare access journey.

Recognizes evolving shifts in market trends and respective effects on provider and patient.

Outstanding customer relationship, interpersonal, communication, and presentation skills with the ability to effectively work with diverse audiences and influence cross‑functionally.

Moderate level of travel required to vendor and home office sites.

Compensation

Base salary range: $210,000 - $257,000 per year. All full‑time employees receive equity and comprehensive benefits including flexible PTO, medical, dental, vision, life/disability insurance, and 401(k) with employer match.
