We are looking for a PT Case Manager to provide and oversee home health services within the community, collaborating with clients, families, and the healthcare team to develop and implement goal-centered care plans while ensuring quality measures and financial considerations. Schedule
This is a full‑time position, Monday – Friday, with an on‑call rotation. Manage your schedule during office hours. Requires travel to client homes to provide 1:1 client care that may take 1–3 hours per client. Coverage Area
Reports to the Blaine, MN office and provides services in Mora and surrounding cities. Benefits

Reflects the company’s mission and values in developing and maintaining professional working relationships. Perform initial and periodic assessments of clients to determine care needs, implement interventions, and evaluate effectiveness. Collaborate with physicians/practitioners and the home health team to develop and update care plans at least every 60 days or more often as needed. Serve as client advocate, ensuring safety needs are addressed, assessing the home environment, developing abuse prevention plans, and reporting suspected vulnerabilities. Meet documentation standards ensuring accuracy, timeliness, and compliance with state and payer/billing requirements. Provide education to the client, family, caregivers, licensed clinicians (as indicated), and unlicensed personnel. Provide direct care to clients of varying ages, acuity levels, and diagnoses in accordance with company policies and the Minnesota Physical Therapy Practice Act. Coordinate client care with family, caregivers, community resources, and other healthcare members. Provide clinical oversight, delegation, and leadership to licensed clinicians and unlicensed personnel. Ensure effective and appropriate financial management of client home health services. Manage work schedule effectively, adapt to unexpected changes, and remain flexible. Physical/Environmental Demands
Client care is provided in individual homes and may require transfers and lifting with adaptive equipment. Must be adaptable to various environments and capable of driving under all weather conditions. Heavy lifting: 50–100 lbs occasionally; 25–50 lbs frequently, 10–20 lbs constantly. Physical demands exceed those for medium work. Qualifications
Currently licensed as a Physical Therapist (PT) in Minnesota, with license free of restriction. Proof of negative TB test (Mantoux, blood test, or chest X‑ray). Access to a dependable automobile for traveling to multiple stops per day. Valid driver’s license and proof of current car insurance if using a personal vehicle. U.S. citizenship or proof of valid Alien Work Permit. Disclose any conviction or criminal history related to healthcare provision; candidates with such convictions will not be hired. Pass initial and ongoing background studies and screenings, including Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities. Skills & Experience
At least 2 years’ experience in a healthcare facility or home care field. Demonstrate critical thinking, adherence to standards and procedures, and appropriate clinical judgments. Work independently with minimal supervision, adapt to change, and manage multiple assignments while prioritizing duties. Build and maintain professional relationships. Excellent written and oral communication; interpersonal communication and teaching skills. Basic computer skills and willingness to learn new software programs; experience with electronic medical records and Microsoft Outlook preferred. Current CPR certification highly recommended. Compensation
The base salary range for this position in the U.S. is $80,000 – $110,000 per year plus productivity, compensation, and benefits. Salary is determined by factors including work experience, skills, certifications, job level, supervisory status, and location.
