Job Summary The Advanced Practice Provider (APP) in the Memory Disorders clinic will provide comprehensive evaluation and management of patients with cognitive and memory concerns. Working collaboratively with physicians, nursing staff, and other interdisciplinary team members, the APP will conduct detailed patient assessments, perform neurological and cognitive screenings, formulate differential diagnoses, and develop evidence‑based care plans.
Responsibilities Managing ongoing treatment, coordinating referrals, educating patients and families about disease progression and care options, and contributing to clinical research or quality improvement initiatives related to memory disorders. The APP will play an integral role in optimizing patient outcomes and supporting the mission of providing compassionate, specialized care for individuals with memory and cognitive impairments.
