Pay Range Base pay range: $81,494.00/yr - $155,459.00/yr
Summary The Intensive Community Mental Health Program (ICMHR) Registered Nurse (RN) is a member of the Behavioral Health Care Service Line whose role is to collaboratively manage, coordinate and provide health care to a defined group of high-risk patients with diagnosed chronic mental illnesses and substance use disorders. Care is provided to patients with the goal of achieving quality patient care by improving clinical outcomes, increasing continuity of care and minimizing health care costs.
Qualifications
English Language Proficiency – a requirement of 38 U.S.C. 7403(f) for direct patient care positions.
Graduate of a school of professional nursing approved by an accrediting agency (ACEN or CCNE), or a candidate currently enrolled in a master’s level bridge program whose coursework is equivalent to a bachelor’s level degree; may obtain temporary appointment pending completion.
Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state, territory, or the District of Columbia.
Foreign‑educated graduates may be accepted if they hold a formal degree equivalency validated by a recognized evaluator (e.g., International Consultants of Delaware). Veterans Housing Administration; references: VA Handbook 5005 Appendix G6.
Graduate Nurse Technician (GNT) exception for candidates who meet basic education but lack licensure; entered at the entry step of the grade with a 120‑day temporary appointment while pursuing licensure.

Preferred: BSN with three years of mental‑health experience; ANCC Psychiatric‑Mental Health Nursing Certification (PMH‑BC) is a plus.
Physical requirements: visual acuity, keen hearing, clear speech, manual dexterity; long periods of walking, standing, stooping, sitting, bending, pulling, pushing; mechanical lifts provided; exposure to infectious patients; can be exposed to combative patients; annual employee health requirements (e.g., TB screening).
Duties
Provide care and counseling of persons or community by applying the nursing process: data gathering, assessment, nursing judgment, evaluation of responses to health problems via case finding, health teaching, and counseling.
Administer medications and treatments prescribed by authorized providers.
Commit to the patient under ANA Code of Ethics for Nurses—individual, family, group, or community.
Accountable to the ICMHR Nurse Manager for high‑quality nursing care.
Problem‑solving and care facilitation: assessment, planning, implementation, evaluation.
Achieve optimum mental and physical health and illness prevention.
Assess and triage medical needs of all patients in the ICMHR program.
Collect and analyze data to improve care, maximize resource utilization and enhance case management.
Adhere to VHA directives, Joint Commission standards, CARF accreditation standards, state licensure standards and VA‑OIG expectations.
Leverage virtual care technology to support the mission of the facility and VA.
Provide services in patients’ homes and community settings (>80% of time).
Collaborate with Primary Care Providers, PACT, and specialty clinic staff.
Advocate for patient needs, attend medical appointments with patients, contribute to treatment planning.
Utilize assessment and screening tools and maintain accurate patient records following confidentiality laws.
Conduct crisis management and seek opportunities to improve practices.
Perform other duties as assigned by the facility, department, or agency.
