Shift: 10:00 AM to 06:00 PM
Travel Pay: 3500.41 (W2: $41/hr. | Stipend: $1860/wk.) Local: $75/hr. on W2
Job Summary: The
Registered Nurse Case Manager
is responsible for coordinating comprehensive care plans that promote effective utilization of healthcare services, enhance quality outcomes, and ensure smooth transitions of care. This role works collaboratively with physicians, social workers, and interdisciplinary teams to provide patient-centered, cost-effective care.
Responsibilities: Conduct comprehensive patient assessments to develop and implement individualized care plans. Coordinate care across disciplines, ensuring continuity from admission through discharge. Monitor patient progress and update care plans to support optimal outcomes. Facilitate utilization review and ensure medical necessity for services provided. Educate patients and families on care plans, treatment options, and post-discharge resources. Support discharge planning to minimize readmissions and promote continuity of care. Collaborate with multidisciplinary teams to align care delivery with patient goals. Accurately document all case management activities within the EMR system.
