Full Time Physical Therapist Case Manager, Level 1 Anticipated Territory : Santa Clara County. Multiple small coverage areas. In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. Responsibilities

Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan. Help patients with mobility, movement, and regain independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure a quality level of care. Evaluate and report patients’ progress to the MD. What you’ll do as a Home Health Physical Therapist Case Manager

Evaluate and treat patients using medically prescribed physical therapy programs. May supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Benefits and Perks

Current unrestricted CA Physical Therapist License Current BLS/CPR Certification Valid CA Driver's License Knowledge of local community resources is required Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care Knowledge of state and federal homecare regulatory guidelines preferred Experience in home health care is highly preferred Experience with high acuity patients is a plus Experience working with an interdisciplinary team is highly desired Case Management fundamentals is highly desired Self-Directed, Empathetic, Critical thinker, Outstanding interpersonal skills, Leadership skills, Organized Compensation

AMD120,000 - AMD135,000 a year. Wage range for Physical Therapist, Level 1: 118,000.00 - 135,000.00 plus 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401k (Up to 4% match). As a practice, the majority of PT Case Managers are hired as PTIs and may be promoted to PTII with corresponding salary and bonus progression. Additional notes

You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs.
