Registered Nurse Case Manager: Home Health
Pay range: $130,000.00 per year - $160,000.00 per year.

All positions require field visits to patients’ homes.
What you’ll do as a Registered Nurse (RN) Home Health Case Manager Utilize your clinical skills to create, implement, and execute comprehensive care plans, promoting wellness and preventing readmission. Oversee patient and family education in wound care, blood draws, tube feeding, tracheostomy care, Foley catheter care, CHF management, and medication use.
Perform a full assessment of the patient and family upon initial visit.
Use assessment data to develop an advanced plan of care, including rehabilitation.
Work one‑on‑one with patients and family to achieve improvement/stability.
Case‑manage the patient’s care episode with an interdisciplinary clinical team.
Prepare clinical notes, orders, and charting within compliant timeframes.
Collaborate with physicians and agency staff to deliver optimal care.
Initiate and administer IV medications (antibiotics, diuretics) safely and effectively in accordance with protocols.

To be a Success as a Registered Nurse (RN) Home Health Case Manager
Current, unrestricted California RN license.
Current BLS/CPR certification.
Valid California driver’s license.
High level of clinical knowledge in home health care preferred.
Experience with high acuity patients is a plus.
Experience working with an interdisciplinary team highly desired.
Case management fundamentals desired.
Previous IV medication infusion experience highly preferred.
Proficient in initiating intravenous therapy.
Self‑directed, empathetic, critical thinker.
Outstanding interpersonal skills, leadership, organization.
Completed computer literacy and competency assessment before scheduling an in‑person interview.
