USD 47,000 – 51,376 per year

Position Overview The Case Manager provides and coordinates client services within a resource-intensive environment, ensuring that homeless residents receive comprehensive support and guidance toward permanent, affordable housing.

Responsibilities

Conduct intake within 24 hours of admission, explaining resident rights, responsibilities and grievance procedures.

Perform psychosocial, vocational/educational and housing assessments and document findings.

Maintain accurate, timely documentation of services and client progress toward housing goals.

Prepare housing applications and assist clients through the application/interview process.

Monitor and assess client behavior and appearance changes.

Facilitate group and individual counseling, workshops, psycho‑education and recreational activities.

Inspect rooms/dorms to evaluate personal hygiene, cleanliness and ADL skills.

Provide comprehensive case management to assigned caseloads.

Develop initial service plans and conduct reviews as needed.

Offer referrals for medical, mental health and community-based social services.

Ensure clients attend meals and maintain appropriate nutrition.

Provide crisis intervention and manage emergency situations.

Assist with building evacuation during fire drills and emergencies.

Monitor medication compliance and collaborate with nursing staff.

Refer clients to community resources as required.

Maintain CPR and First Aid certification.

Keep up‑to‑date client charts in compliance with DHS, OTDA, and SUS standards.

Facilitate discharge/transfer processes and documentation.

Attend staff meetings, case conferences, group supervision, training, and escort clients to housing interviews.

Comply with SUS employee policies, corporate compliance policies, and NYS Department of Social Services rules, regulations, and statutes.

Work schedule includes weekends and evenings.

Qualifications

Required:

High school diploma or GED; ability to work with a culturally diverse population.

Preferred:

Bachelor’s degree in Human Services or related field; 1–2 years experience with homeless populations; experience in residential settings, group work, tenant organization, case management, referral and/or community outreach.
