Registered Nurse – Home Health / Hospice Location:
Seattle, WA Shift:
8-hour shifts Job Summary: The Registered Nurse provides skilled home health and hospice nursing care to patients of all ages in their place of residence. This role is responsible for patient assessment, development, and management of the Plan of Care, coordination with the interdisciplinary team, supervision of support staff, and ensuring high-quality, compliant, patient-centered care. Key Responsibilities: Assess home health and hospice patients in both stable and emergent situations, identifying physical, psychosocial, and environmental needs. Develop, implement, evaluate, and update the Plan of Care to ensure quality and continuity. Document all visits, changes in condition, interventions, and communication accurately and promptly. Incorporate physical, psychological, social, spiritual, and cultural factors into care planning. Provide patient and family education on medications, care techniques, safety, and self-care. Supervise HHAs, STNAs, and LPNs/LVNs to ensure consistent, high-quality care. Assess and manage patient pain; educate families about pain management techniques. Coordinate referrals and ensure payer compliance for assigned cases. Participate in on-call rotation and provide after-hours patient support. Serve as a liaison between patients, families, physicians, and the interdisciplinary team. Attend team meetings and perform other related duties as required. Required Skills & Experience: Current RN license in practicing state CPR certification Valid driver’s license, reliable transportation, and auto insurance 1–2 years of RN clinical experience 2+ years of home health experience (required) Homecare Homebase experience (required) OASIS experience (required) Strong assessment, wound care, IV care, venipuncture, and medical/surgical skills Ability to organize, prioritize, and work independently Ability to maintain confidentiality and work as part of a team Preferred Qualifications: CHPN certification Education: Graduate of an accredited ADN or nursing diploma program (required) BSN preferred Pride Global offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance, and employee discounts.
