Base pay range $86,421.00/yr – $159,605.00/yr

Summary The Registered Nurse Care Manager (RNCM) is responsible for organizing and directing team members to achieve desirable patient outcomes through day‑to‑day and/or telephonic case management. The RNCM functions in the Primary Care Clinic which utilizes a Patient Aligned Care Team (PACT) model. The RNCM uses the nursing process and evidence‑based practice to collaborate with the Core Team in developing the patient‑driven care plan.

Qualifications

Basic Requirements:

English Language Proficiency – in accordance with 38 U.S.C. 7403(f); a person must be proficient in basic written and spoken English to serve in direct patient‑care positions.

Graduate of a professional nursing school approved by the appropriate accrediting agency, accredited by ACEN or CCNE.

OR individuals attending a master’s‑level bridge program may qualify if coursework is equivalent to a bachelor’s‑level degree in nursing.

Upon achievement of a state license, the applicant may be appointed on a temporary basis and later converted to a permanent appointment.

Foreign‑school graduates: must possess a current, full, active, and unrestricted registration to be considered a qualified nurse. Credit for foreign education higher than an associate degree requires formal equivalency validation from a recognized equivalency evaluation accepted by VA (e.g., ICD).

Graduate Nurse Technician (GNT) Exception: candidates meeting education requirements but lacking licensure may be appointed at the entry step of the applicable grade on a 120‑day temporary appointment while pursuing licensure (may be extended up to two years on a case‑by‑case basis).

Grade/Level Scope Education requirements vary from Nurse I to Nurse III, ranging from an Associate Degree to Doctoral degrees, with corresponding years of professional nursing experience as defined in the online assessment.

Preferred Experience: 1–2 years Ambulatory Care or Med‑Surg experience, Triage Experience (telephone and/or face‑to‑face), Care Coordination Experience/Certification, Ambulatory Care Certification.

Physical Requirements: heavy lifting of 45 lb or more, straight pulling (6 hrs), pushing (2 hrs), reaching above shoulder, use of fingers, walking (6 hrs), standing (2 hrs), repeated bending (6 hrs), visual requirement (computer screen), basic color distinction, hearing (aid may be permitted), visual acuity 20/30.

Duties

Demonstrates clinical knowledge in assessing, planning, implementing, documenting, and evaluating care for a designated group of patients across the continuum of care.

Triage and apply a collaborative team approach in identifying, analyzing, and resolving patient‑care problems.

Collaborates with patients to assess and identify needs, issues, care goals, and resources for discharge, post‑hospital recovery, and health maintenance/wellness.

Maintains current knowledge of multidisciplinary resources, programs, and services, referring patients for community resources as appropriate.

Utilizes problem‑solving methodologies and advances clinical knowledge to resolve system or process issues that impact patient care.

Assists with navigation of health systems and intervenes or refers as needed to facilitate the achievement of patient outcomes.

Provides culturally respectful care, honoring patients’ personal values.

Assists with the development, assessment, and monitoring/evaluation of comprehensive care plans to ensure efficient, effective, and coordinated care that meets wellness standards.

Coordinates an interdisciplinary approach to ensure physical, psycho‑social, and environmental needs and resources are delivered to achieve patient goals.
