Description The Registered Dietitian contributes and participates as a health care team member to provide needed nutritional counseling service to individuals and families; serves as a nutrition consultant to staff; assists in the development of nutritional educational materials; participates and takes charge of the nutrition component of agency projects.
Essential Job Functions
Develops and implements age appropriate, linguistically correct, and culturally sensitive tools for nutritional counseling on both normal and therapeutic diets.
Assesses patient needs, establishes priorities, designs, develops, recommends, updates, implements and evaluates programs, curriculum, materials, protocol, procedures and standards for nutrition education/health education.
Conducts Medical Nutrition Therapy (MNT) upon physical referrals.
Documents accurately and thoroughly in the Participants’ electronic health record in accordance with industry and departmental standards including time requirements.
Contributes and assists in the development and periodic updates of the Nutrition EHR templates for charting.
Implements nutrition care to include follow up and referrals to government, community and/or other NEMS for continuity of nutrition care.
Develops meal plans as appropriate to meet member’s lifestyle, socio-economic background, age, culture, diagnosis and religious affiliation.
Develops and maintains Nutrition protocols up to date.
Establishes relationships with community agencies for patient referrals.
Assists in the development of handouts on nutritional educational materials.
Utilizes Epic appropriately and efficiently and serve as content and workflow expert for resolving issues and proposing solutions in Epic.
Performs home visits as necessary.
Performs other job duties as required by manager/supervisor.
Qualifications Required
Bachelor’s degree in Nutrition or related field. Master’s degree preferred
Registered Dietitian (RD) in the Academy of Nutrition and Dietetics in good standing
Current BLS and First Aid required
Proficient in computer usage
Preferred
Minimum one year of experience working with a frail and/or older adult.
2+ years experience in nutritional assessment, coaching and counseling patients
2+ years of work experience as RD in community agencies. Serving a diverse population with different cultural needs preferred
Physical
Occasional ten key typing
Constant sitting through out an entire work day shift
Rare occasions of pushing/pulling objects under 10lbs
Language
Must be able to fluently speak, read and write English.
Fluency in Chinese (Cantonese) is preferred.
Fluency in other languages is an asset.
