Physical Therapist Case Manager: Home Health
Full‑time position in Santa Cruz County (primary location).
The role requires on‑site visits to patients’ homes; remote work is not available unless specifically noted.
Base Salary:
$120,000 – $135,000 per year, plus incentive bonus ($10K annual bonus, paid quarterly).
Bonus structure may increase to $20K for higher‑level positions.
Responsibilities
Perform home evaluations and develop individualized care plans.
Collaborate with physical therapy assistants (PTAs) to implement and monitor care plans.
Assist patients with mobility, movement, and regaining independence.
Communicate and coordinate with physicians and interdisciplinary team members to ensure quality care.
Evaluate and report patient progress to medical directors.
Qualifications
Current unrestricted California Physical Therapist license.
Current BLS/CPR certification.
Valid California driver’s license.
Knowledge of local community resources.
Understanding of human behavior, culture, and health care impacts.
Knowledge of state and federal home‑care regulatory guidelines (preferred).
Experience in home‑health care (highly preferred).
Experience with high‑acuity patients (plus).
Experience on interdisciplinary teams (highly desired).
Case management fundamentals (highly desired).
Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
