Job Description The
Saville Center Advanced Practice Provider
works under general supervision and in accordance with APP standardized procedures/protocol.
Counsels and educates patients and families and monitors patient progress from initial visit to long‑term follow‑up. Coordinates comprehensive cancer screening and risk assessments and advises patient on prevention guidelines. Responsible for ongoing clinical management of the patient during their visits but works in collaboration with the patient’s referring provider as well as other members of the patient’s care team to ensure optimal patient experience and outcomes.
Responsibilities
Ensure patient information, visit history, refer outs, lab follow‑up, phone calls, and consults are carefully and completely documented in the electronic medical record.
Conduct in‑depth interviews with patients and/or family, construct comprehensive medical histories, perform initial and periodic physical examinations, and explore patient’s interpretation of health needs. Identify and treat patients’ diseases.
Participate in research efforts to include enrolling patients in cancer screening clinical trials (e.g. patients with environmental exposures such as firefighters and first responders).
Directly perform or order appropriate diagnostic studies and interpret finished results.
Instruct and counsel patients and families regarding preventive care, medical problems, psychological problems, and use of prescribed treatments and drugs.
Identify and record pertinent progress of patients, update and summarize charts, change orders when appropriate and notify the responsible physician of changes in the patient’s condition.
Work with members of interdisciplinary team to ensure optimal patient experience and outcomes.
Educate/coach fellow staff members including RNs and medical assistants.
Participate in community/programmatic events per a defined work standard (e.g., 8 events per year).
Minimum Qualifications
Education:
Master’s in Advanced Clinical Practice or graduate of a post‑master’s certificate program for nurse practitioners, or certificate from an accredited program that is endorsed by the Council of Nurse Anesthesia Educational Programs/Schools, American College of Nurse Midwives, American Association of Colleges of Nursing, or National League for Nursing. For physician assistants, graduate of an accredited Physician Assistant program and certification by the National Commission on Certification of Physician Assistants.
Experience:
No experience required. 2 years Advanced Practice experience preferred.
Certification:
Basic Life Support (BLS) upon start.
Licensure:
Licensed Physician Assistant or Nurse Practitioner; if Nurse Practitioner: RN Compact License (depending on state of primary residence) and a license (or eligibility for) as a nurse practitioner or physician assistant in the Commonwealth of Virginia and a “Practice Agreement” signed by supervising physician(s), a written “Scope of Practice/Protocol” signed by the supervising physician, and a “Drug Enforcement Administration License” (if prescribing controlled substances).
Payer Credentialing:
Ability to successfully complete the credentialing process by third‑party payors and to enroll as a CMS Medicare/Medicaid payer.
Medical Staff Credentialing:
Successful completion of credentialing for Medical Staff membership and appropriate clinical privileges sufficient to perform all required services.
Spanish language proficiency
strongly preferred.
