Ascension medical Group–Koeller St Clinic, Oshkosh, WI is looking for a Family Practice Advanced Provider to join our elite team (2 MD/DOs and 4 Nurse Practitioners – fully staffed). Thriving clinic environment. Providers see 12–18 patients per day. Conveniently located off Hwy 41 in Oshkosh.

Responsibilities Ascension–Wisconsin is looking for a Board Certified/Board Eligible Advanced Practice Provider for our Primary Care–Family Medicine Team.
This is a great opportunity for the right candidate to partner with Ascension–Wisconsin in Oshkosh, to provide medical services to our patients.

Responsibilities:
In collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.
Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care.
Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

Requirements Licensure / Certification / Registration:
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing is also accepted. Licensure is dependent on state associate works in. Verified by local credentialing office.
Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Minnesota Board of Nursing OR current home state license, if considered multi–state/Compact State, also accepted. Licensure is dependent on state associate works in. Verified by local credentialing office.
Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred. Verified by local Credentialing Office.
Education:
Master's degree of Nursing required.
