Speech Language Pathologist (SLP)

We are a leading home health agency serving patients across the Bay Area. We are now hiring a Home Health Speech Therapist/Language Pathologist (ST) based in the Gilroy/Morgan Hill Area.
Pay and Benefits Base pay range: $110,000.00/yr - $125,000.00/yr. Additional incentive bonus program, 401k with up to 4% match, generous PTO, medical, dental, life insurance, flexible spending accounts, pet insurance and more.
Responsibilities In partnership with the Case Manager, you will provide skilled services to assist patients with breathing, speaking and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and member of the interdisciplinary care team.

Qualifications
Current unrestricted California Speech Therapist License
Current BLS/CPR Certification
Valid California Driver’s License
Knowledge of individual development, human behavior, and cultural influences on health care
Knowledge of state and federal home‑care regulatory guidelines preferred
Experience in home health care highly preferred
Experience with high‑acuity patients a plus
Experience working with an interdisciplinary team highly desired
Case management fundamentals highly desired
Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organization
Computer literacy and competency assessment required pre‑interview
