$35,000 in annual bonus opportunity

Your Responsibilities Conduct comprehensive medical and functional assessments of patients in skilled nursing and rehabilitation facilities. Collaborate with an interdisciplinary team?including physicians, physical therapists, and occupational therapists?to develop and monitor individualized care plans. Manage and evaluate patient progress, addressing medical barriers to recovery and optimizing outcomes. Participate in daily clinical rounds and coordinate with nursing staff and facility administrators to ensure quality care delivery. Document visits and care plans using the
Doctor Now
EHR, integrated with
PointClickCare . Provide ongoing communication with the primary care team, contributing to chronic care management and readmission prevention programs. Maintain communication with regional clinical supervisors to ensure clinical excellence and team collaboration. Qualifications Licensed
Nurse Practitioner (NP)
or
Physician Assistant (PA)
in
Massachusetts . Strong interpersonal, communication, and clinical assessment skills. Experience or interest in
Physical Medicine & Rehabilitation (PM&R)
preferred. Ability to work independently while collaborating with a multidisciplinary team. Passionate about patient-centered care and improving outcomes. What We Offer Compensation:
