Monday–Friday | 8:00 AM–5:00 PM Contract:

6-Month Contract Pay:

$42/hr (W2)

Our team is supporting a Bay Area healthcare institution in hiring an

Aftercare Coordinator (TBI) / Patient Navigator

to support their pediatric trauma program. This role plays a key part in helping children and families navigate the recovery journey following a

traumatic brain injury (TBI) .

The ideal candidate has hands-on experience working with

pediatric populations ,

TBI-affected families ,

behavioral health/psychosocial support , and

multidisciplinary care teams . Candidates who meet

at least 2 of these 4 experience areas

will be considered. Licensure accepted:

LCSW, LMFT, RN, NP, PA, or similar licensed healthcare provider.

Please note: All candidates must be fully vaccinated against COVID-19 as required by the client.

Key Responsibilities Patient Navigation & Family Support Serve as the primary point of contact for families throughout the TBI recovery process. Provide emotional support, education, and resource navigation tailored to each patient. Coordinate transitions between inpatient, outpatient, and community services. Support school reintegration and long-term adaptation following TBI.

Program Coordination Partner with trauma leadership to design and implement program components. Build and maintain collaborative relationships across internal and external partners. Support a multidisciplinary care model to ensure continuity and integration of services.

Project Management & Administrative Support Develop and maintain project plans, tracking deliverables and resource needs. Prepare reports, presentations, and program documentation. Maintain confidentiality and accuracy in sensitive records.

Education & Training Assist in developing educational content for families and staff. Deliver trainings and act as a subject-matter resource in TBI survivor support.

Quality Improvement & Community Engagement Participate in trauma-focused process improvement initiatives. Support community events (occasional evenings/weekends). Contribute to data collection and program evaluation.

What We’re Looking For Candidates should bring experience in

at least 2

of the following: Pediatric care Support for children/families impacted by TBI Behavioral health or psychosocial support Working within a multidisciplinary clinical environment

Additional Requirements Ability to work 100% onsite between two nearby clinical locations in Oakland. Strong communication, empathy, and coordination skills. Flexibility to support occasional evening/weekend events.
