to join our community-based family medicine clinics in
Florence and Coolidge, AZ . This full-time role provides a consistent, flexible four-day schedule and the opportunity to deliver full-spectrum primary care to patients of all ages in a supportive, mission-driven practice. Who We?re Looking For You?re a confident, compassionate provider who thrives in a
high-volume ,
collaborative outpatient setting . You?re committed to delivering evidence-based, patient-centered care and value continuity, health education, and community health improvement. Key Responsibilities Perform comprehensive physical exams and health assessments Diagnose and manage acute illnesses and chronic conditions Order and interpret labs, imaging, and diagnostic studies Prescribe medications and implement treatment plans Educate patients on disease prevention, treatment adherence, and lifestyle modification Conduct minor in-office procedures (e.g., wound care, injections, suturing, immunizations) Document all patient encounters accurately in the EHR Coordinate referrals and support continuity of care Collaborate with physicians, RNs, and allied staff Participate in quality improvement initiatives, case reviews, and care planning Qualifications Active and unrestricted
Arizona license
as an NP or PA Graduate of an
accredited NP or PA program Master?s degree in Health Sciences or related field For NPs: Approval by the
Joint Board of Medical Examiners and Board of Nursing
and
active Arizona RN license Minimum
2 years of experience
