to join its team in delivering high-quality, community-based family medicine. This position is ideal for a provider seeking
autonomy ,
patient continuity , and a
predictable weekday schedule

$100,000 ? $140,000 annually (based on experience) Bonus Plan:
Percentage-based incentive linked to collections Benefits Package Includes: Medical, dental, and vision insurance Paid vacation time and separate sick leave 401(k) with employer contributions CME stipend and reimbursement for licenses/certifications Employer-paid malpractice insurance Note: This position does not include a sign-on bonus or relocation assistance. Key Responsibilities Perform physical exams, preventive care screenings, and wellness visits Diagnose and treat minor injuries and acute illnesses Manage chronic conditions such as diabetes, hypertension, and COPD Order and interpret labs, imaging, and diagnostic tests Counsel patients on health education, lifestyle changes, and treatment adherence Coordinate referrals and ensure follow-up care is completed Maintain accurate records within the electronic health record (EHR) system Contribute to internal quality initiatives and peer review activities Qualifications Active and unrestricted Arizona license as an NP or PA Graduate of a U.S.-based accredited Nurse Practitioner or Physician Assistant program (Note: Programs based in Puerto Rico are not accepted) DEA certification (or eligibility prior to start) Able to credential with Medicare, Medicaid, and major commercial insurance plans No history of malpractice or disciplinary actions Preferred:
