Monday - Friday schedule with flexible start and end times
Work autonomously in skilled nursing facility setting
Board-certified or board-eligible in Internal Medicine, Family Practice, or Geriatrics
Focus on post-acute care including rehab and recovery patients
Partnership opportunity available after initial employment period
Physician-owned medical services group with Hartford Healthcare partnership
Experience preferred but new graduates with post‑acute interest considered
