About the Program NYC Health + Hospitals

is launching the HEAL Program to provide comprehensive medical services to young people in City custody. The role will offer 24/7 medical care across two secure detention sites, two school‑based health centers, and limited pharmacy services at seven non‑secure sites (group homes). Services include comprehensive medical and dental screenings, vaccinations, routine testing, immediate care responses, and referrals within and outside the

NYC Health + Hospitals

system.

Pay Range $70.05/hr – $93.40/hr (based on skills and experience; discuss with recruiter).

Location & Schedule Work locations: Crossroads Juvenile Center and/or Horizon Juvenile Center. Schedule: 21 hours per week – Day, Overnight, and Weekend.

Responsibilities

Provide medical care to ACS adolescents while maintaining safety and security measures.

Take patient history, identify needs, and make diagnoses.

Develop and implement treatment plans using diagnostic data.

Perform complete well‑care examinations, including physical and gynecological histories.

Write prescriptions, inform patients about medications, and obtain parent/guardian consents when needed.

Complete medical forms and documentation related to patient care.

Track and make referrals to other agencies and providers, ensuring follow‑up care.

Assist with phlebotomy and administer immunizations as needed.

Distribute, refill, remove, and discontinue medications for patients.

Complete court orders and all other ACS‑mandated paperwork.

Evaluate and treat ACS employees only on an emergent basis.

Provide health education on preventive medicine, chronic disease counseling, parenting, prenatal care, nutrition, family planning, medication usage, and stress management.

Administrative duties:

Act as on‑site supervisor of Patient Care Associates when in the field.

Teach PCAs about patient care and triage, improving patient flow, and communicate with the Director of Nursing regarding clinic status.

Participate in case conferences discussing patient care.

Assist the Medical Director in developing and implementing medical protocols and forms.

Initiate and participate in research related to health care in collaboration with the Medical Director.
