Position Summary:
The Clinical Manager will assume overall responsibility for the implementation, delivery, coordination and supervision of hospice services provided on a day-to-day basis. This position supports established goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.
Essential Duties & Responsibilities:
* Serves as a member of the leadership team for Peoples Hospice division. * Assumes overall responsibility for oversight and management of the clinical services provided by the interdisciplinary team (IDT) within the homecare and facility setting. * Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with the Medical Director regarding any questions about an individual's eligibility for services. * Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate. * Conducts supervisory visits to allow for observation of clinical performance of assigned team members. * Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course * Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing Plan of Care * Attend case conference meetings with hospice personnel to facilitate coordination of care and discussion of Interdisciplinary Group involvement. * Conducts quarterly record reviews and communicates findings and recommendations to Director of Nursing and hospice personnel. * Analyzes, reviews and approves assigned employees' time worked and exception hours recorded for the previous week and resolve any discrepancies on a weekly basis. Reviews and approves employee time entry for payroll process. * Assists in the screening and interviewing process of new employees and makes recommendations for the employment of individuals. Coordinates the clinical orientation of new employees. * Assists in the planning, implementation, and evaluation of in-service and con¬tinuing education programs. Assists in formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range * Complies with accepted professional standards and principles. * Collaborates with leadership team in strategic planning activities, identifying opportunities to improve services and better meet client needs. * Collaborates with operations and clinical staff to ensure the intake process meets the needs of the patient, family and physician/other referral source. * Participates in orientation, training, on-going education programs, instructing and coaching to ensure hospice personnel understand and can clearly articulate the hospice philosophy from intake through bereavement. * Collaborates with Clinical Staff to develop clinical educational program/in-services for conversational and classroom presentations. * Other duties as directed.
Requirements: Education/Experience
* Graduation from an accredited RN program. BSN preferred. * A minimum of 3 years RN experience * At least 2 to 3 years of hospice experience. * A minimum of 3 years of supervisory experience. * Will consider an equivalent combination of relevant education and experience approved by the VP of Operations and Hospice Administrator.
Licenses, Certifications and/or Registrations
* Current RN State of Florida licensure. * Current Hospice and Palliative Care Nurse (CHPN) preferred. * Valid driver's license and valid auto insurance. * Current CPR certification.
