Job Summary The Physical Therapist, reporting to the Director of Rehabilitation, is responsible for administering physical therapy services within the scope of practice, ethical, and professional standards. This role involves treatment planning, assessments, therapeutic interventions, and collaboration with an interdisciplinary team. The Physical Therapist contributes to program development, quality improvement, and efficiency enhancement in an interdisciplinary team environment. The position also includes providing guidance and supervision for therapy assistance and students as per licensure requirements, ensuring the highest quality therapy services, and maintaining positive relationships with facility employees, residents, and family members.
Responsibilities Continuing Education
Contribute to in-services as scheduled and maintain communication with the therapy team.
Monitor and ensure the currency of licenses and certifications.
Participate in competency peer reviews as assigned.
Ensure orientations are completed.
Assist the Supervisor in identifying educational needs for the therapy team.
Conduct education and in-services for the therapy team.
Encourage the development of student programs.
Ensure licensure and certifications are current.
Professionalism
Maintain a professional appearance, demeanor, and communication with patients, families, clients, coworkers, and others.
Adhere to the professional dress code as established by the facility and Rehab Solutions.
Facilitate effective communication with patients and facility representatives.
Adhere to HIPAA regulations regarding patient confidentiality.
Compliance
Familiarize yourself with company Policies and Procedures.
Promote adherence to company Policies and Procedures, the Code of Conduct, facility policies, and standard operational practices.
Client Relations
Participate in pre-admission assessments.
Promote positive interactions with facility personnel.
Maintain professionalism and positive communication with customers.
Patient Care and Documentation
Complete screening according to policy and procedures.
Complete patient-specific documentation timely and in appropriate areas.
Conduct patient assessment after establishing Physician orders.
Properly file or record patient-specific documentation in the appropriate medical record.
Establish long-term and short-term goals based on assessments.
Update goals in a timely manner.
Establish goals and interventions based on assessment results.
Document goal achievement timely for progress reports, including progress made and the need for treatment and family education.
Collaborate with team members and physicians regarding goals and interventions.
Provide patient and family education according to the treatment plan and therapy's role.
Coordinate with the facility for discharge planning.
Quality Management and Meetings
Participate in facility quality management and program meetings.
Attend interdisciplinary team meetings.
Assist in interpreting therapy reports to facility personnel.
Contribute to facility operations and compliance through quality review data.
Participate in survey preparedness and safety committee activities.
Ensure compliance with safety standards and hazard reduction efforts.
Other Responsibilities
Contribute to patient care and treatment.
Productivity to be determined by Supervisor.
Facilitate team meetings for problem-solving regarding therapy programs.
Work with the Director of Rehabilitation to set and achieve departmental goals.
Assist the therapy department in carrying out shared departmental functions, including treatment, goals, and special projects.
