A home care agency is seeking a motivated, detail‑oriented, and compassionate Patient Retention Coordinator to support patient satisfaction and continuity of care. The ideal candidate thrives in a fast‑paced environment, excels at communication, and is passionate about helping others.
Responsibilities
Build and maintain positive relationships with existing patients and caregivers to promote satisfaction and ongoing services.
Conduct follow‑up calls to ensure patient needs are met and address any service‑related concerns promptly.
Collaborate with nursing, intake, authorizations, and scheduling teams to minimize service interruptions.
Identify and report patterns that may impact patient retention or satisfaction.
Participate in retention initiatives and campaigns to improve patient engagement and loyalty.
Requirements
Excellent interpersonal, communication, and problem‑solving skills.
Empathetic, patient‑focused, and proactive approach.
Experience in customer service, healthcare coordination, or home care patient retention.
Bilingual English/Spanish highly preferred.
Ability to work in a busy office environment and perform detailed tasks using office equipment.
