We make house calls – enabling access to quality wrap‑around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

We offer an array of medical and chronic care services and we're backed by SCAN, a not‑for‑profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

Homebase Medical is expanding rapidly into the

Huntington Park, Cudahy, Downey, Paramount, Bell, Bell Gardens and Pico Rivera CA areas.

We are currently searching for an experienced full‑time

Advanced Practice Clinician (Physician Assistant or Nurse Practitioner)

to join our expanding house call practice. You will be responsible for primary care in the home setting, Board and Care, and Assisted Living Facilities.

Key Duties

Conduct

Annual Wellness Visits

focused on patient education.

Deliver

Chronic Care Management

to improve long‑term outcomes.

Perform comprehensive in‑home visits including:

Medication review & chronic illness management.

Medical history updates & physical exams.

Labs, diagnostic screenings, and post‑hospital follow‑ups.

Primary care management & palliative care services.

Qualifications

Active

Nurse Practitioner

or

Physician Assistant

license in the State of California.

Board certification (Nurse Practitioner or Physician Assistant).

DEA licensure.

Prior experience preferred (but passion and commitment are just as important).

Stable income + unlimited earning potential.

Typical Annual Earnings:

$132,000 – $212,000+

Guaranteed Annual Base Salary:

$68,640.

Per‑patient rate:

$90 for every patient seen.

Earning security:

Each pay period, you're paid whichever is higher – your base salary or your per‑patient total.

$500/month no‑show protection.

Mileage reimbursement per IRS guidelines.

Internet stipend.
