Compensation Base pay range: $89,000.00/yr - $110,000.00/yr.

What You'll Do To provide nursing services in accordance with applicable regulations, policies, and guidelines to foster the highest degree of quality care for all residents; to provide oversight of the Clinic/AL CNA staff; and to assist the Clinic Manager in oversight of the Assisted Living Program, Clinic Operations, and the Clinic Licensed Nurse (LN) staff.

Responsibilities

Participates in admissions, discharges, and evaluation procedures for all residents, ensuring appropriate assessments and documentation are completed in a timely manner and meet State and Federal requirements.

Conducts, schedules, reviews initial/annual nursing assessments, administers professional nursing services, and monitors nursing care services and treatments administered to all residents.

Assists in the development, maintenance, and revisions of the residents’ service plans or service plan updates.

Oversees medication administration, ensuring adequate ordering, receiving, documentation, safe storage and disposal of physician-prescribed medication for residents; conducts regular review of medication documentation.

Works in conjunction with the Clinic Manager and Clinic Licensed Nurses to ensure resident assessments are completed in accordance with State and Federal regulations.

Responds to the Emergency Call alarms from apartments or other areas in facilities; assesses residents’ physical and mental conditions, performs first aid and takes other appropriate action as indicated.

Communicates effectively with healthcare providers regarding orders, nursing services, and changes in residents’ conditions.

Promptly reports concerns or complaints to the Clinic Manager; communicates effectively and courteously with residents, families, and visitors.

Performs monthly skill checks for Licensed Nurses in identifying training needs.

Provides oversight of incident reports for completion and conducts root cause analyses of incidents with implementation of corrective actions as needed; provides oversight of the daily activities of the Clinic/Assisted Living Licensed Nurses/CNAs.

Coordinates and conducts ongoing training and in-service programs for the Licensed Nurses/CNA staff, as needed.

Coordinates and conducts Nursing Shift Reports, as needed.

Coordinates and conducts new Licensed Nurse staff employee orientation and training programs.

Assists the Clinic Manager in reviewing the CNA staff’s documentation and effectiveness of services.

Assists the Clinic Manager in carrying out Performance Management activities for Licensed Nurses, including the creation of individualized staff development goals.

Participates and assists during emergency drills and situations such as fire, fire alarms, hurricanes, and other possible emergencies as outlined in the Disaster Response Playbook.

Participates in Quality Assurance/Quality Improvement programs, including generating utilization reports/routine reports and analyzing or reviewing trends.

Ensures compliance with all of the organization’s policies and procedures, as well as state and federal laws and regulations.

Ensures that all resident rights and responsibilities are protected and enforced; establishes and maintains a close liaison with residents and families.

Assesses employee injuries or emergencies, performs first aid and takes other appropriate action as indicated.

Maintains sufficient supplies and monitors proper operation of medical devices and equipment.

Performs annual employee and resident PPD skin testing/screening; administers other immunizations ordered by physicians in accordance with our policies and procedures.

Communicates effectively with other departments regarding residents’ care concerns or issues.

Assists the Clinic Manager and Social Worker with information gathering and completion of Long-Term Care Insurance forms.

Supervisory Responsibilities Provides oversight of Licensed Nurses staff, including training employees; planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws.

Education / Experience Bachelor’s degree (B.S.) from a four-year college or university and a minimum of 2–3 years experience and/or training; or an equivalent combination of education and experience.

Certifications, Licenses, and Registrations

Current State of Hawaii R.N. license

Current First Aid and CPR certification

Computer Skills Proficiency in using a computer and applicable software programs, including the Microsoft Office Suite (Excel, Outlook, PowerPoint, Word) and designated electronic medical records software (Health MedEx Vision). Capacity to learn and master other applicable software applications.

Physical Demands While performing the duties, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently stands, walks, sits, tastes or smells. The employee occasionally climbs or balances and stoops, kneels, crouches, or crawls; pushes and pulls. The employee must frequently lift and/or move up to 50 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
