Develop effective treatment plans and obtain necessary approvals from referring physicians. Provide patient care according to the physician-approved treatment plan. Communicate regularly with supervisors and other healthcare team members regarding patient progress, challenges, and plans. Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed. Engage in in-service training programs for facility staff. Maintain accurate documentation of evaluations, daily treatment notes, weekly progress reports, and recertification documentation in accordance with the Speech and Language Pathology Board State Practice Act and regulatory requirements. Document treatment changes per facility policies and procedures. Educate patients' families or nursing staff on maintenance programs and provide caregiver training in preparation for discharge. Participate in discharge planning to ensure a smooth transition for patients. Serve as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologists and student programs. Comply with the Speech-Language Pathology and Audiology Bureau State Practice Act and facility policies. Build positive, effective relationships with patients, residents, families, staff, and professional colleagues. Required License or Certification
