Base pay range $144,094.00/yr - $187,325.00/yr

Summary The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice - top of licensure - and serves as a mid-level provider - working independently with prescribing privileges as outlined in the individual's practice area-based scope of practice. The CPS provider delivers evidence-based comprehensive medication management (CMM) through direct patient care and as part of the interdisciplinary team providing Oncology/hematology (referred to as oncology in document) care.

Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements

Citizenship - Citizen of the United States

Education - Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree

Licensure - Full current and unrestricted license to practice pharmacy in a State - Territory - Commonwealth of the United States (i.e., Puerto Rico) - or the District of Columbia

Grandfathering Provision - All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title - series and grade held - including positive education and licensure/certification/registration that are part of the basic requirements of the occupation

Grade Determinations: GS 12-This recruitment is for the GS-13 grade level

Experience or Education - In addition to the basic requirements - candidates must meet one of the following: 1 year of experience equivalent to the next lower grade level - OR Completion of an ACPE-accredited Pharm.D. program

Preferred Experience - PGY2 residency or Clinical Pharmacist in Oncology experience of 1 year

Key Knowledge, Skills & Abilities

Knowledge of professional pharmacy practice

Ability to communicate orally and in writing to patients and health care staff

Knowledge of laws, regulations, and accreditation standards related to scheduled and non-scheduled drugs

Skill in monitoring and assessing outcomes of drug therapies

Expert understanding of regulatory and quality standards for their program area

Ability to solve problems and coordinate responsibilities to maximize outcomes

Duties

Prescribes, monitors, and adjusts medications related to cancer and supportive care, reviews and recommends anticancer therapy adjustments based on labs and toxicity assessments, obtains pertinent medical history and accurate anticancer medication history from patients and chart review.

Implements follow-up on treatment actions as supported by lab interpretations, orders imaging as directed by clinical plan and within scope.

Communicates results with providers, orders and interprets precision medicine testing (pharmacogenomic – somatic – germline).

Provides patient and caregiver instructions and education; uses available population health tools to identify and monitor patients on oral anticancer therapies.

Ensures referral of complex or unstable patients to higher-level care when needed; addresses other care needs and refers to other services as appropriate.

Performs medication reconciliation and aligns with policy.

Provides cancer and supportive care management and CMM for complex patients with chronic disease exacerbated by oncology therapy.

May treat other comorbid conditions in alignment with scope of practice.

Accepts urgent or same-day visits, participates in multidisciplinary tumor boards, researches drug information questions, and documents clinical interventions in the electronic health record.

Provides formal and informal education to pharmacy, medical, nursing, and other staff.

Participates in design and completion of research studies and quality improvement projects involving drug therapy or patient care improvement.

Work Schedule Monday – Friday 7:30 am – 4:00 pm or 8:00 am – 4:30 pm (subject to change based on service needs). Telework: Not Authorized. Virtual: This is not a virtual position.
