Position Title: RESIDENTIAL SERVICES SPECIALIST – La Selva Residential
Employer: Momentum for Health
Schedule: Sunday to Thursday 1:45 PM – 10:15 PM
Union Pay (SEIU Local 521): Step 1 – $35.49 /hr; Step 6 – $39.19 /hr
Shift Differentials:
6.25% overtime for overnight shifts (11 pm–7 am)
4.75% overtime for evening shifts (5:30 pm–10:59 pm)
4.75% overtime for weekend shifts (Saturday – Sunday)
Benefits: Defined contribution up to $15,433.08 per year (prorated for part‑time); 12 paid holidays; up to 25 days PTO; 6 sick days; generous retirement plan with 4% employer match after 1 year; health, dental and vision plans available; plus other cafeteria plan options.
Principal Duties and Responsibilities
Provide milieu and/or mental health, rehabilitation and case management services to support clients in daily living (also referred to as “milieu” services).
Develop and implement individual service and treatment plans.
Facilitate psychiatric consultations and group sessions by attending psych meetings, taking notes and scheduling clients.
Coordinate medical follow‑up: help clients obtain primary care physicians, arrange appointments and transportation, and assist with medical issues.
Assist clients with insurance benefits: determine coverage, help with applications, liaise with insurance carriers, provide benefits training and answer questions from staff.
Serve clients and families by linking to professional and community resources; coordinate care with other programs; inform case managers and probation agents; advocate with outside agencies; facilitate housing interviews; educate families on mental health issues.
Handle counseling especially during conflict and crisis; monitor independent living skills; assess stability; approve daily and weekend passes; coach problem‑solving, socialization and coping skills; write 5150 holds for high‑risk clients; support budgeting and financial planning; offer medication education.
Participate in discretionary resource management for client needs.
Maintain clinical records and documentation per agency, payor and regulatory requirements.
Perform administrative admissions and discharge paperwork; complete admission and discharge summaries.
Enter and update clinical documentation, including case notes and treatment plans.
Oversee medication issuance: monitor inventory, fax prescriptions, transcribe orders, complete medication sheets.
Perform general office work: electronic and telephonic communications, timecard processing.
Provide transportation in personal or agency vehicles for clients and supplies.
Offer on‑call and floor coverage; participate in after‑hours rotation, shift exchanges and debriefings; provide consultation and backup during client crises.
Perform first aid in emergency situations.
Collect reports and data; send daily census reports to Central Intake.
Audit and review documents; conduct monthly medical record reviews.
Attend staff, program, agency and community meetings and trainings; conduct trainings as needed.
Ensure compliance with health and safety or other regulatory requirements.
Maintain program facilities in a clean and well‑kept condition.
Qualifications
Education:
High School diploma or GED with 5 years related experience (E)
Associate’s degree with 3 years related experience (E)
Bachelor’s degree with 2 years related experience (E)
Master’s degree with 1 year related experience (E)
Experience:
Human service setting (E)
Residential mental health setting (2+ years) (E)
Experience with emotionally disturbed individuals (D)
MHRS qualifications or greater experience (E)
Eligibility for MHRS certificate (D)
Relevant volunteer or internship experience (D)
Abilities:
