DDS/DMD Degree : Possess a Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) degree and an active, unrestricted license in the state of Illinois.

Physical Stamina : Capable of lifting and carrying up to 45 pounds when necessary.
