Job Summary The Care Partner supports the RN by providing patient care, performing ADLs, ambulation, vital signs, and potentially advanced technical skills such as phlebotomy, simple treatments, dressing changes, oral suctioning, and catheter management. Responsibilities include clerical duties, maintaining supplies, patient transport, and effective communication in a fast‑paced environment.
Core Responsibilities And Essential Functions
Provide patient‑centered care in collaboration with RN and health‑care team.
Assist with ADLs, hygiene, nutrition, elimination, and comfort measures.
Perform basic patient care functions and advanced skills per unit needs.
Respond to call lights and telephone promptly.
Assist with patient admission, data entry, and charge reconciliation.
Maintain confidentiality, HIPAA compliance, and patient dignity.
Participate in hourly patient rounding and safety interventions.
Support infection control, hand hygiene, and PPE use.
Transport supplies, medications, and equipment.
Document patient care and communicate concerns via EMR.
Required Minimum Education
High School Diploma, GED, or General‑Preferred.
Required Minimum License(s) And Certification(s)
BLS – Basic Life Support (or equivalent).
Additional License(s) And Certification(s)
Graduation from a state‑approved CNAs or Patient Care Tech program preferred.
Required Minimum Experience Must be 16 years of age or older.
Required Minimum Skills
Ability to follow detailed directions.
Strong interpersonal and communication skills.
Clear verbal and written communication, including documentation.
Ability to work independently and as part of a team.
Minimum age of hire is 16.
