Director of Rural Health Transformation (Durational Project Manager)

We are seeking an experienced public health leader to direct Connecticut’s Rural Health Transformation Program (RHTP), a $100 million federal grant that will improve the health of residents in rural areas across the state. The position is based in the Secretary of the Department of Social Services (DSS) Commissioner’s Office and will terminate at the end of the RHTP grant period (September 30 2031), unless funding ends sooner.
Position Highlights
Full‑time (40 hr/week), 1st shift (Mon – Fri, 8:00 a.m. – 4:30 p.m.)
Hybrid role consistent with the State of Connecticut Telework Policy
Primary location: Hartford, Connecticut (travel primarily in‑state)
Duration: Until the end of the RHTP grant period (September 2031)
Responsibilities
Lead and manage the CT Rural Health Transformation Project, providing strategic oversight and project management for planning, launching, budgeting, coordination, risk mitigation, evaluation, and communication.
Perform financial, monitoring, and reporting oversight, including data‑sharing, analysis, and coordination of all grant‑related activities.
Prepare and submit required reports, applications, and other submissions to the federal government and partner agencies.
Serve as the main point of contact with the federal government and coordinate grant duties within DSS and other state agencies.
Ensure compliance with federal grant terms and applicable federal and state requirements.
Oversee sub‑projects within RHTP and supervise associated staff.
Provide leadership and support to a newly created rural health transformation advisory council.
Recruit, hire, and procure RHTP‑funded contractual partners as needed.
Qualifications
Demonstrated success in high‑visibility leadership roles.
Experience managing large, complex federal grant projects, including oversight, reporting, compliance, evaluation, and coordination with multiple partners.
Strong stakeholder engagement skills and ability to serve as a point of contact for internal and external inquiries.
Familiarity with and demonstrated interest in health or social services programs, especially related to rural areas.
Preferred Qualifications
Experience in rural health initiatives or rural workforce development.
Proven track record of securing and managing multi‑million‑dollar public‑sector grant dollars.
