Position Summary We have an exciting opportunity to join our team as a Field Nurse – Brooklyn – Home Care Division – SIGN ON BONUS – $5,000. In this role the successful candidate provides safe, quality therapeutic age‑appropriate nursing care using evidence‑based practice, coordinates care with the interdisciplinary team to promote optimum patient outcomes and satisfaction.
Job Responsibilities
Perform other duties as assigned.
Complete a comprehensive nursing assessment including but not limited to physical, functional, psychosocial, emotional, cognitive, sexual, cultural, age‑related, environmental, spiritual, transpersonal and economic factors in a systematic and ongoing process while honoring the uniqueness of the patient.
Derive the diagnoses and needs from the assessment data, validate the diagnoses, and collaborate with the patient, family, caregivers and physician to develop an individualized plan of care to attain expected quality outcomes.
Coordinate care by interacting and collaborating with other members of the healthcare team to optimize patient outcomes, patient satisfaction and health care goals.
Deliver direct nursing care, ongoing assessment, teaching and training to patients/caregivers that preserves and protects patient autonomy, dignity, rights, values, and benefits using evidence‑based nursing practice.
Identify deficits and barriers to effective knowledge understanding of education provided to patients. Employ strategies to optimize patient engagement. Demonstrate caring behaviors toward patients, family and other caregivers.
Complete and maintain accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education intervention and evaluation according to federal, department of health and the joint commission requirements.
Assist agency leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participate in agency quality assessment performance improvement initiatives.
Minimum Qualifications Current Registered Professional Nurse in New York. Bachelor’s degree in Nursing required. Basic Life Support certification through the American Heart Association. Fluent written and spoken English. 1‑2 years experience in an acute‑care facility or home health, hospice, or long‑term home health care program. Excellent communication, interpersonal, customer service, critical thinking skills. Working knowledge of computers.
Required Licenses Registered Nurse License – NYS, Basic Life Support Certificate.

NYU Langone Health provides a salary range for the role of $110,966.74 – $160,466.74 annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range listed does not include bonuses, incentive, differential pay or other forms of compensation or benefits.
