Seeking a Primary Care Advanced Practice Provider to join a collaborative team in New Windsor, NY.
This full-time or part-time position focuses on delivering comprehensive primary care services, including prevention, wellness, acute care, and chronic disease management.
The role requires working onsite, utilizing generic electronic medical records systems, and participating in team-based care.
Requirements
Licensed as a Nurse Practitioner or Physician Assistant in New York State.
Ability to provide comprehensive primary care services.
Experience in preventive care, wellness visits, and chronic disease management.
Proficient in prescribing medications and therapies within scope of practice.
Skilled in ordering and interpreting diagnostic tests.
Strong communication skills for patient education and engagement.
Ability to collaborate with a multidisciplinary care team.
Qualifications
Minimum 10-15 years of primary care experience preferred.
Current knowledge of primary care clinical guidelines and best practices.
Participation in continuing medical education relevant to primary care.
