Cleerly has created a new standard of care for heart disease through value‑based, AI‑driven precision diagnostic solutions. Our technology goes beyond traditional measures by enabling comprehensive quantification and characterization of atherosclerosis (plaque buildup) in each of the heart arteries. It is supported by over a decade of large clinical trials identifying key findings that increase the risk of heart attacks. At Cleerly, we collaborate digitally and use Google Workspace, Slack, Confluence/Jira, Zoom, and other tools. While we are mostly a remote company, travel is required for some team meetings and cross‑functional projects, typically once per month or once per quarter. Some roles (e.g., sales or external‑facing positions) may require up to 90 % travel.
About the Opportunity We are seeking an experienced Imaging Integration Backend Engineer to join the core technical team of our next‑generation enterprise‑level imaging platform. This individual will design, implement, and deploy core platform components, with a key focus on integrating best‑in‑class cardiac applications into the platform. The Senior Software Engineer will collaborate closely with Product, Program, SRE, SQA, and other Software Engineers, as well as development partners, to build the next generation product for heart disease diagnosis.
Responsibilities
Own Data Connectivity: Implement and own core data movement patterns connecting hospital PACS/VNAs, third‑party applications, and external AI endpoints to our platform Hub.
Deliver Robust Ingest/Write‑back: Provide ingest and write‑back paths, including DICOMweb, proxying/presigned access, site networking playbooks, and edge connector patterns for legacy/on‑prem vendors to ensure predictable and supportable deployments.
Full Cycle Participation: Actively participate in the full software development life cycle (SDLC) from requirements analysis through testing, release, and maintenance.
Drive Code Quality: Perform rigorous code reviews, ensure excellent unit test coverage, and actively drive bug triage and resolution.
Design & Document Solutions: Design, document, and demonstrate technical solutions with flowcharts, diagrams, and clear code comments.
Development Contribution: Develop and maintain APIs, integrations, and libraries that support the platform.
Implement Observability: Integrate logging, metrics, and tracing tools to meet the platform’s observability goals.
Qualifications
6–10+ years of professional experience integrating medical imaging systems in clinical production environments.
Strong experience integrating PACS/DICOM systems with cloud‑hosted services (AWS preferred) using secure transfer patterns (DICOMweb, presigned URLs, private networking/VPN).
Hands‑on DICOM knowledge (C-STORE, C-FIND, C-MOVE) and expert‑level troubleshooting capabilities.
Experience deploying and managing on‑prem “edge” components (VM/appliance deployment, outbound‑only connectivity).
Design and implement reliable ingestion and write‑back paths (acks, retries, queueing, replay).
Understanding of core DICOM identifiers and spatial concepts.
Comfortable with imaging toolkits (dcm4che, Orthanc, pydicom, fo-dicom, etc.).
Proven ability for secure handling of PHI (minimization, encryption, retention/deletion practices).
Operational mindset focused on site support readiness via logs, metrics, and trace IDs.
Nice‑to‑have
IHE profiles (e.g., Invoke Image Display) and enterprise imaging integrations.
Familiarity with DICOM‑SR/SEG/PR content and validation tooling.
Familiarity with HIPAA/HITRUST security requirements.
Experience with imaging workflows.
Education
Bachelor’s degree in Computer Science, Engineering, or another advanced technical degree (preferred).
TTC :
$159,000 – 198,000
Total Target Compensation includes base pay, variable pay, commission, bonuses, etc. Pay depends on experience, technical depth, geographic location, and alignment with internal peers.
