Description
The Medical Case Coordinator is responsible for the development, implementation and supervision of all systems involving medication administration, health and medical oversite, and training needs for the assigned program(s). Hours are primarily in the day however evening and weekend availability may be required based on the needs of the program. This includes assisting in overseeing the proficiency and delivery of medical related services for the assigned program. The Medical Case Coordinator will assist with the oversite of medically complex clients including but not limited to attending appointments, assisting with hospital admissions and discharges, coordinating care with the health care providers and medication and treatment administration as needed.
The Medical Case Coordinator is responsible for maintaining the training records for medical related trainings such as MAP, OSHA, First Aid, CPR and Unique Medical Needs of the clients. The Medical Case Coordinator works directly with the Director of Health Care Services in ensuring that all individuals receive appropriate support and care for all health care issues.
The Medical Case Coordinator will maintain all MAP regulatory requirements for the assigned program(s) though monitoring and medication audits and will work in conjunction with Quality Assurance (QA) to ensure that all MAP site locations are functioning under current State regulatory guidelines. The Medical Case Coordinator will assist QA during licensing reviews in the areas of MAP and Medical Training. This is accomplished by conducting reviews of medication administration practices at regular intervals that include but are not limited to:
a) Administering medications and treatments per LHCP orders
b) Observing Certified and Licensed staff administering medication to verify that they are following correct MAP policies, procedures and protocols
c) Verifying that the prescribing practitioner's orders are on site, current and properly transcribed.
d) Verifying that medications dispensed by the pharmacy corresponds with those medications prescribed by the practitioner; verifying that all medication is being administered in accordance with the prescribing practitioner's written order; verifying that medication is being properly procured and stored.
e) Maintaining EHR including, but not limited to, creating health encounter forms, APE packets, medication orders, uploading encounter forms, med orders, relevant health information, etc.
f) Updating HCSIS Health Care Record as needed.
g) Verifying that the program site has a current Massachusetts Controlled Substance Registration (MCSR) on-site and appropriate documentation of all Certified and Licensed staff administering medication at the site.
h) Reviewing documentation of medications being administered at the site to ensure that proper clinical protocols are in place and are being observed, e.g., monitoring vital signs, blood glucose monitoring, and appropriate practitioner notification; and,
i) Reviewing Medication Occurrence Reports (MORs) to determine areas where further training or other intervention may be needed
j) Coordinating and attending health and medical appointments, attending ISP/DDS/Stakeholder meeting for assigned individuals.
k) Ensuring nursing scheduling is complete and assisting with filling open shift.
l) Covering on call for assigned programs
Primary Responsibilities Also Include : Will assist with the oversite of medically complex clients including but not limited to attending appointments as needed, assisting with new admissions, hospital admissions and discharges, coordinating care with the health care providers This may include evenings and/or weekends as needed. Will perform medication and treatment administration as needed and will train licensed or MAP certified staff as needed to meet the needs of the clients. Work collaboratively with QA and the Director of Health Care Services in auditing all MAP licensed sites to ensure full compliance with current State regulatory requirements. Obtain and/or maintain MAP licenses for designated sites. Provides ongoing Quality Assurance by monitoring a MAP site's medication administration system to ensure compliance with Amego, DPH; and DDS; regulations Work collaboratively with Staff Development and Training in scheduling and conducting various medical trainings including, but not limited to the following: OSHA, vital signs, signs and symptoms of illness, med. side effects, epi-pen, seizures, unique medical needs, etc. Assess training curriculum and work collaboratively with Director of Health Care Services in updating and or modifying content when needed. Assist in maintaining training data base as it pertains to medical and/or health related topics. Work collaboratively with QA in auditing medical services and overall quality at designated sites. This includes assisting QA in the preparation and coordination of all licensing reviews. Provide additional support and training to staff during the MAP testing periods to increase success in passing. Maintain data on pass/fail rates and determine if changes in training delivery are needed to increase passing rate for the agency. Assesses issues and the technical functions related to medication administration that may be unique to individuals served at a particular site e.g., side effects, liquid medication, vital signs, blood glucose monitoring and takes steps to ensure that Certified staff are properly informed, educated and/or competent with regard to such issues. Works collaboratively with program supervisory staff to identify and address individually and collectively, medication administration related remedial training needs for Certified staff. Administers medication and treatment as needed, this may include assisting with day program as needed. Work collaboratively with PC/PD/SRD, administrative staff, direct care staff, health care providers and guardians during admission and discharge process as well as in ensuring medical and health services are met for all individuals. Maintaining EHR including, but not limited to, creating health encounter forms, APE packets, medication orders, uploading encounter forms, med orders, relevant health information, etc.; updating HCSIS Health Care Record as needed. Attend MAP trainer meetings and/or other relevant trainings or meetings for the agency. Maintain up to date information on all State and Federal regulatory requirements: MAP, DDS, DPH, OSHA, CARF. Maintain On-Call responsibilities. Ensuring nursing scheduling is complete and assisting with filling open shifts. Maintain flexible schedule to meet all unique training needs for the agency. This may include evenings and/or weekends. Ensures timely submission and follow-up of all Medication Occurrence Reports (MOR's) and med error forms and retraining. Ensure the promotion of Amego's name and image within the community. Complete any additional duties as assigned by the Director of Health Care Services when needed.
Salary Range:
RN: $80k-89k yearly
LPN: $31-$33/hr

Qualifications
•Must be a Licensed Nurse. RN preferred, but may be waived in favor of a strong clinical background..
•Current license to practice in Massachusetts or must obtain a license to practice in Massachusetts within 90 days.
•At least two years of nursing experience required. Experience with severe population preferred.
•Demonstrated clinical skills and knowledge.
•Secure Massachusetts MAP Trainer Status (for RN's)
•Demonstrated experience in managerial duties and systems. Exceptional organizational skills required.
•Effective communicator, with experience developing decision-making processes and as a facilitator.
