Base pay range $35.00/hr - $50.00/hr
Job Description Job Requirements
Bachelor of Arts, Science Degree, Master’s Degree, or an Associate Degree in an allied health field, or comparable skilled nursing experience deemed adequate by Regional Director of Operations.
Current state licensure and/or certification/registry, as required.
Physical Requirements
Ability to sit for extended periods of time, walk through the department and office and lift objects weighing up to ten pounds. Must be able to climb stairs, stoop and reach when performing daily work.
Must also meet physical requirements for job description of held licensure. For example, Physical Therapist.

Job Relationships Responsible to and reports to the Director of Operations.
Job Responsibilities
Manage and administer all aspects of rehabilitation services and maintain a clinical caseload.
Recruit, retain, orient, and train all rehabilitation service personnel and conduct performance reviews.
Promote positive employee relations through open communication and problem solving.
Oversee operations of rehabilitation services at the facility level and monitor personnel compliance with all standards, taking corrective action when needed.
Work with finance personnel to reconcile billing and ensure accuracy.
Ensure all program standards, including MDH SNF standards and other overseeing agencies, are met.
Identify community program development needs and implement appropriate programs.
Develop clinical affiliation programs with educational institutions.
Investigate and assist in resolving staff, service, and resident-related issues as requested by the Administrator and/or Executive Director.
Review quality and appropriateness of the total services delivered and attend facility meetings as requested.
Assess resources and supplies available in the community; track MDS Assessment Period Data and communicate it to appropriate community personnel.
Review resident-specific PDPM categories per assessment period and coordinate changes in MDS data according to community procedure.
Utilize tracking systems and implement changes indicated.
Ensure compliance with data entry for resource utilization and CPT coding daily and monitor treatment minutes.
Participate in quality assurance programs as required.
Adhere to confidentiality standards for residents and the organization.
Maintain appropriate relationships and rapport with coworkers, supervisors, residents, family members, community personnel, and administration.
Project a positive and professional image at all times.
Generate special reports/data to assist the Administrator, Executive Director, and Team Leader in assessing the efficiency and cost effectiveness of rehabilitative services.
Maintain punctuality, coordinate schedule to achieve maximum results of rehabilitative services operation within the facility.
Maintain a safe and clean work area and comply with community safety, Infection Control, Universal Precautions, and OSHA standards.
Perform other duties as assigned to effectively perform the responsibilities of the position in the best interest of Northwoods Rehab.
Assume responsibility for ongoing continuing education and professional development.
Adhere to job responsibilities for job description of held licensure (e.g., Physical Therapist).
