Job Title Full Time Occupational Therapist (OT) – Home Health
Anticipated Territory San Francisco County
What You’ll Do In partnership with the Case Manager you will evaluate and address the needs of individuals in their homes, allowing them to live safely and productively. Your responsibilities include:
Home safety and medication management assessment
Wheelchair seating and positioning
Pain management and falls prevention
Management of chronic diseases and dementia care
Family caregiver training and education
Patient advocacy and interdisciplinary team collaboration
Qualifications
Current unrestricted California Occupational Therapist license
Current BLS/CPR certification
Valid California driver’s license
Knowledge of local community resources
Understanding of individual development, human behavior, and cultural influences on health care
Knowledge of state and federal home‑care regulatory guidelines (preferred)
Experience in home health care (highly preferred)
Experience with high‑acuity patients (a plus)
Experience working with an interdisciplinary team
Case management fundamentals (highly desired)
Self‑directed, empathetic, critical thinker with outstanding interpersonal and leadership skills
