Position Summary HousingPlus is hiring a Justice Works Outreach Intake Coordinator to join our growing company. The Justice Works Outreach Intake Coordinator is a key part of the Housing Plus Justice Works program and is expected to represent the Justice Works program for a range of settings and audiences. The Outreach Intake Coordinator will report directly to the Justice Works Senior Program Director. The Coordinator will work closely with the referring partners, make weekly visits to Rikers Island and collaborate with all Justice Works subcontractors. The Coordinator will manage all referrals and intake tasks as they relate to the Justice Works program, ensuring all referrals have been appropriately screened based on the Justice Works’ eligibility requirements and the subcontractors’ individual program requirements. In order to adhere to Justice Works contractual goal of 159 filled units the Coordinator will monitor the flow of referrals, admissions, and discharges.
Competencies / Skills
Exceptional Communication Skills:
The Outreach Intake Team explains the Justice Works program and procedures to subcontractors, referring partners and clients, communicating in a way that is easy to understand, asking the right questions, and recording accurate details to keep the referrals on track.
Attention to Detail:
An important responsibility of the Outreach Intake Coordinator is the preparation and management of referral data for the MOCJ monthly report and LL79 information; consistent accurate reporting strengthens Housing Plus’s reputation for reliability.
Technical Proficiency:
Managing client data, report files and communication efficiently requires the right tools: AWARDS, Excel and other data‑entry software and communication platforms.
Problem‑Solving Skills:
Strong problem‑solving skills help the Outreach Intake Coordinator assess problems, ask the right questions and keep the referral process moving smoothly.
Remote Workforce Integrity:
Upholding ethical standards, transparency, and accountability within a decentralized work environment through clear expectations, open communication, and consistent leadership check‑ins.
Time Management:
The Outreach Intake Coordinator will handle multiple inquiries, paperwork and follow‑ups throughout the day to keep everything on track.
Duties / Responsibilities
Review and screen all Justice Works referrals to ensure they meet the eligibility requirements of the MOCJ contract and subcontractor program requirements.
In partnership with the Justice Works Senior Program Director assess the needs of the referred resident and submit appropriate referral to subcontractors to ensure community partners meet the clients’ needs.
Responsible for updating the Justice Works occupancy report and admission and discharge log; track the availability of beds among the Justice Works consortium.
Maintain ongoing contact with ATI, public defenders and other community agencies concerning referrals for potential residents, and participate in regular Referring Partner meetings with providers as scheduled.
Manage pending referral wait‑list.
Participate in individual supervisory sessions and case reviews with the Senior Program Director and other staff as required.
Attend agency‑wide meetings and staff training.
Perform other duties as assigned by the senior leadership team.
Required Skills / Abilities
Outreach and intake work experience, preferably working with women experiencing homelessness histories, behavioral health needs and justice impact history.
Meaningful understanding and commitment to housing‑first, harm‑reduction and trauma‑informed care.
Excellent communication, presentation and coordination skills, with demonstrated ability to coordinate outreach‑intake referral process, resolve related problems and overcome barriers to successful completion of Justice Works contractual goals.
Knowledge of and the ability to adhere to professional code of ethics and conduct.
Confident level of self‑awareness and ability to give and receive feedback well.
Competent technology & management skills, including proficiency with Microsoft Office and client database management systems.
Education and Experience
Associate degree required; Bachelor’s degree preferred.
High School Diploma with 5+ years of related intake experience.
3–5 years experience in social services and/or nonprofit services with particular value placed on New York City‑based experience.

Work schedule: Monday‑Friday 8 AM‑4 PM; weekly visits to Rikers Island.
