We are seeking a registered nurse with a passion for person-centered dementia care and an entrepreneurial spirit to lead the Chicagoland division of Renewal Memory Partners.

In this role, you’ll lead a small but mighty team of Care Managers, Care Coordinators, and Care Partners in the delivery of person-centered care to people living with dementia. You’ll also take a leading role in growing our practice—building relationships with community partners and identifying strategic opportunities to expand our network.

This position is ideal for a nurse with a heart of gold and big ambitions—someone who is excited to work in a creative, entrepreneurial environment, making a difference and prospering along the way.

Business Development

Serve as Renewal Memory Partner's ambassador at various community and networking events throughout the Chicagoland area

Sustain and expand our network of referral sources in the community

Build strategic partnerships that support our growth and enhance our quality of care

Plan community events that educate professionals and family caregivers in best practices

Client Care

Conduct home (re)assessments with our clients

Serve as a trusted point of contact for our clients and their families

Directly supervise the provision of any skilled care our clients receive through per-diem nurses and other skilled staff

Care Partner Training and Support

Evaluate Care Partner performance; provide continuous coaching and guidance to support our Care Partners day-to-day

Oversee our Care Partner training and development program

Conduct select trainings with our Care Partners, community partners, and families

Compliance

Ensure that the agency remains in compliance with all applicable IDPH regulations pertaining to a home services and home nursing agency

Qualifications

RN with active IL license

Passion for dementia care; passion for serving LGBTQ older adults

Outstanding client services skills – capable of working within a concierge environment and building relationships with sophisticated clientele with high expectations

Knowledge of Teepa Snow’s Positive Approach to Care and other dementia best practices a plus.

Experience in homecare a plus

Compensation

Salary + performance-based bonuses + benefits (PTO, health/dental/vision, 401K with matching employer contributions, and more)

Pay: $90,000.00 - $110,000.00 per year

This position includes travel , both domestically and internationally (Philippines)
