Our client is a healthcare technology company focused on transforming chronic and neurodegenerative care through proactive, patient-centered engagement. Their platform enables early detection, continuous support, and personalized care management at home—helping patients stay connected between visits and improving long-term outcomes.
They partner with neurologists and clinics across the Bay Area and beyond to deliver Principal Care Management (PCM) and Chronic Care Management (CCM) programs that enhance patient well-being while generating compliant revenue for their clinical partners.
Role Overview They are seeking an experienced and motivated Registered Nurse (RN) to oversee care delivery operations and ensure the highest standards of clinical quality and compliance. In this role, you will supervise Licensed Practical Nurses (LPNs), review and sign off on care plans, and ensure adherence to Medicare PCM/CCM guidelines. You’ll serve as a key bridge between patients, clinical partners, and operations, ensuring proactive, coordinated care and a consistently positive patient experience. This is a Bay Area-based position with flexibility for hybrid or remote work depending on patient and clinic needs.
Key Responsibilities
Supervise and support LPNs in delivering care coordination, patient engagement, and documentation.
Review, approve, and sign off on care plans and clinical documentation.
Conduct quality assurance reviews to ensure compliance with Medicare PCM/CCM standards.
Serve as the clinical liaison between the organization and partner clinics, facilitating effective communication and issue escalation.
Mentor and train LPNs to promote ongoing professional development and quality care delivery.
Collaborate with clinical operations and technology teams to refine workflows and improve care processes.
Engage patients and caregivers periodically to assess care quality and address escalated clinical needs.
Maintain accurate time-based activity logs for compliant CPT billing and documentation.
Uphold the organization’s commitment to compassionate, proactive, and patient-centered care.
Qualifications
Active, unrestricted RN license in California (multi-state licensure a plus).
2+ years of experience in chronic disease management, neurology, or care coordination.
Experience supervising or mentoring clinical staff (LPNs, MAs, etc.).
Strong communication and leadership skills with a focus on patient-centered care.
Proficiency with EMRs and digital health platforms.
Highly organized and detail-oriented with a commitment to compliance and quality assurance.
Preferred Experience
Knowledge of Medicare PCM/CCM documentation and billing requirements.
Experience with neurodegenerative conditions (e.g., Alzheimer’s, Parkinson’s, dementia, MCI).
Background in telehealth, hybrid-care models, or outpatient neurology.
Experience with quality improvement or clinical operations initiatives.
Multi-state licensure.
Location Bay Area (San Francisco, Berkeley, Palo Alto). On-site preferred, with flexibility for hybrid or remote collaboration.
Compensation & Benefits
Salary: $130,000
