Registered Nurse – Chronic Care Management (Neurology Focus)
Hybrid role with flexibility for partial remote work. Monday–Friday, 9:00am–5:00pm. Compensation up to $130,000 annually DOE.
As the RN, you will oversee a small care delivery team, ensuring quality execution of Principal Care Management (PCM) and Chronic Care Management (CCM) programs for patients across the Bay Area.
You’ll supervise Licensed Practical Nurses (LPNs), review and approve care plans, maintain clinical quality standards, and ensure compliance with Medicare PCM/CCM documentation. The RN bridges patients, partner neurologists, and internal operations—delivering proactive, coordinated, compassionate care.
Responsibilities
Oversee and mentor LPNs and care coordinators in delivering chronic care support.
Review and approve care plans and clinical documentation.
Conduct quality assurance audits to ensure compliance with Medicare PCM/CCM standards.
Act as clinical liaison with partner neurologists and healthcare teams.
Participate in workflow optimization and process improvement initiatives.
Engage patients and caregivers to assess and enhance care quality.
Qualifications
Active, unrestricted RN license in California (multi-state license a plus).
2+ years of experience in chronic disease management, neurology, or care coordination.
Prior experience supervising or mentoring clinical staff.
Detail-oriented, organized, and committed to clinical excellence and compliance.
Proficient with EMRs and digital health tools.
Preferred
Knowledge of Medicare PCM/CCM documentation and billing.
Experience with Alzheimer’s, Parkinson’s, or dementia populations.
Background in telehealth, hybrid care, or outpatient neurology.
