Falmouth, MA. Qualifications
Graduate of an accredited LPN program; current, active MA LPN license required. BLS / CPR and AED certification required. Strong knowledge of nursing/medical practices, long-term care standards, and regulatory compliance. Leadership and supervisory abilities with a teamwork-focused approach. Excellent communication, documentation, and organizational skills. Ability to lift a minimum of 50 lbs and meet all physical/sensory demands of the position. Key Responsibilities
Supervise and direct daily functions of nursing assistants in alignment with facility guidelines. Assist in shift planning and coordination of nursing services and activities. Complete and maintain resident documentation for admissions, transfers, discharges, treatments, and care plans. Accurately transcribe and carry out physician orders; administer medications and treatments per protocol. Monitor and document resident responses to care, reporting changes to physicians and supervisors. Perform skilled nursing services (IV/IM meds, catheterization, tube feedings, wound care, suctioning, colostomy care, specimen collection, etc.). Conduct vital signs monitoring and routine assessments. Ensure adequate medication, supply, and equipment inventory. Provide patient/family health education and orientation for new residents and families. Maintain confidentiality of all resident information in compliance with HIPAA and residents’ rights.
