Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end‑of‑life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life‑threatening diagnosis through death and bereavement.

Provider Relations RN Liaison
will provide education about services to medical providers within assigned territory and meet immediate admission needs of patients.
Schedule Monday - Friday; 8:30am - 5:00pm
Location Stanford Hospital (Palo Alto), including Redwood City to San Jose
Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities
Develop relationships, maintain regular contact, and resolve issues with referral sources including physicians, hospitals, nursing facilities, residential care facilities for the elderly, senior housing, and community service agencies to ensure ongoing quality of care and referrals.
Determine patient eligibility for hospice care.
Admit patients, obtain informed consent(s) for service, inform patient/family of rights and responsibilities.
Assist with the implementation and coordination of services required for the discharge of patients from hospitals into agency’s hospice program.
Communicate with Intake and Admissions department regarding hospice referral/admission.
Assist Director of Provider Relations in negotiating or redefining contract provisions as necessary and in accordance with relevant laws and regulations.
Education/Experience
AA or B.S. degree in Nursing from an accredited Nursing School.
Minimum of one year of experience in a home health/hospice setting.
Certificates, Licenses, Registrations
Current license as a registered professional nurse in the State of California.
Public Health Nursing certification highly desirable.
HPNA certification preferred.
Pay Range $58.90 - $73.65
