Post Acute Care Physician / Primary Care Physician - Berlin, VT
Seeking Board Certified Geriatricians, Internists, and Family Practitioners to deliver comprehensive on‑site care in post‑acute, rehabilitation, long‑term care, and assisted living facilities.
Provide face‑to‑face primary care services to diverse geriatric patients, from short‑term rehab to complex cases.
Participate in monthly educational sessions and clinical outcomes reviews.
Collaborate with Nurse Practitioners and Physician Assistants to enhance patient care.

Qualifications required: current medical licensure, D.E.A. certification, and board eligibility or certification. Positions are available full‑time or part‑time, with in‑person work expected.
