Post Acute Care Physician / Primary Care Physician - Providence, RI Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on‑site medical care in post‑acute, rehabilitation, long‑term care, and assisted living settings. Deliver care to a range of patients from short‑term rehab to complex geriatric cases, with a focus on improving access, removing barriers, and achieving optimal clinical outcomes.

As a team member, you'll offer face‑to‑face primary care, participate in educational sessions, and collaborate with nurse practitioners and physician assistants.
