Our Mission The Critical Time Intervention (CTI) Program is an evidence-based, phase-based, time-limited program designed to assist patients in accessing needed services during critical transition periods.

Purpose of Position Licensed Mental Health Professional (MHP) is a professional who works within their scope of practice in mental health. MHPs are Care Managers but in addition to Care Management tasks, they resolve clinical and rehabilitative needs, when within scope of practice, that impacts the individual’s ability to meet recovery goals.

Responsibilities

Travel throughout the New York metropolitan area to provide therapeutic interventions for both mental health and substance use disorders.

Educate individuals about mental health, treatment, and recovery.

Teach skills for coping with specific symptoms and stress management.

Develop crisis management plans, such as relapse prevention plans, identification/recognition of early warning signs and rapid intervention strategies.

Collaborate with families and social supports as indicated/requested by the participant for treatment needs.

Provide community and family support treatment services.

Document client progress to maintain a permanent record of client activity according to established methods and procedures.

Follow the established CTI policies & procedures, agency policies and maintain appropriate professional standards.

Use individual client data to inform clinical decision-making and identify areas for quality improvement; use data to track client progress over time.

Share client data with clients to help facilitate shared-decision making and keep client informed of their progress.

Other related duties or projects as assigned.

Requirements

The licensed clinician must possess and maintain an active license with a master’s degree in the human services field such as social work or mental health counseling. LMSW or LMHC preferred.

Experience using culturally competent treatment approaches.

Knowledgeable about the psychosocial aspects of illness and health.

Proficient with word processing, electronic health records, and electronic communications.

Well organized with strong time management skills.

Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues.

Knowledgeable with regard to the availability of community facilities and resources for referral, transfer, and discharge planning.

Crisis intervention skills.

Able to meet program standards in documenting in client records.

Comfortable and competent working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities.
