We are currently seeking a dedicated Restorative Licensed Practical Nurse (LPN) to join our team. Through individualized care plans and restorative programs, we focus on helping residents maintain or regain their highest level of physical, mental, and psycho social well-being. ‍⚕️ Position Overview: Restorative LPN Nurse The Restorative LPN Nurse assumes daily responsibility and accountability for the restorative nursing care of assigned residents. This role coordinates restorative services, collaborates closely with nursing leadership and therapy departments, and ensures restorative care is delivered in alignment with each resident’s comprehensive assessment and plan of care. ✅ Key Responsibilities of the Restorative LPN: Oversee and coordinate the restorative aspect of nursing care for assigned residents Confer with the Director of Nursing Services and licensed nursing staff regarding restorative care needs Participate in the MDS/RAI process and care planning related to restorative nursing services Initiate therapy and evaluation requests and respond to referrals from residents, physicians, or nursing staff Serve as a liaison between the nursing department and therapy services Evaluate residents and review medical records to identify normal and abnormal findings Recognize when residents require physician evaluation, supervision, or direction Implement and maintain facility policies and procedures related to restorative nursing care Communicate restorative care plans and policies to physicians, residents, and family members Utilize resources efficiently to support optimal resident outcomes Qualifications for the Restorative LPN Nurse: Current Licensed Practical Nurse (LPN) license in good standing Must meet all applicable federal and state regulatory requirements Demonstrated basic nursing skills and clinical knowledge related to elderly and disabled populations Proven leadership and supervisory skills Strong assessment and communication abilities Patient, tactful, enthusiastic, and positive attitude toward residents and families Passion for promoting independence and quality of life through restorative care Why Join Our Restorative Nursing Team?
