We specialize in rehabilitation services, helping individuals recover, regain strength, and thrive in a supportive, patient-centered environment. ‍⚕️ Position Overview: Licensed Practical Nurse (LPN) We are actively seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our interdisciplinary healthcare team. In this role, the LPN provides hands-on nursing care, supports rehabilitation goals, and works closely with RNs, physicians, and therapists to ensure residents receive safe, dignified, and high-quality care. ✅ Key Responsibilities of the LPN: Provide direct patient-centered nursing care in a long-term care and rehabilitation setting Administer medications and treatments as prescribed by physicians Monitor residents’ conditions, including vital signs, and promptly report changes to the supervising RN or physician Assist residents with activities of daily living (ADLs) such as bathing, dressing, and feeding when needed Collaborate with the interdisciplinary care team to support rehabilitation and recovery plans Maintain accurate, timely, and compliant documentation in the electronic medical record (EMR) Provide emotional support and education to residents and their families Promote a safe, clean, and therapeutic environment by following infection control protocols Qualifications for the Licensed Practical Nurse (LPN): Current and valid Licensed Practical Nurse (LPN) license New grads encouraged to apply! Experience in long-term care, skilled nursing, or rehabilitation preferred Strong clinical skills, including medication administration and basic wound care Excellent communication, documentation, and teamwork skills Compassionate, patient-focused approach to care BLS/CPR certification required Why Join Our LPN Team?
