Palliative Care Physician opening Äì Hartford, Connecticut Position Details
- Full-time Hospice and Palliative Care Physician role
- Practice in a progressive, physician-led environment with nationally respected colleagues
- Network includes seven acute care hospitals, large academic and surgical hospitals, thriving community teaching hospitals, and a multi-specialty group of 1,000+ providers
- Schedule: 48 on-call weeks per year, Monday 7 a.m. ÄìFriday 7 p.m., including mandatory in-person attendance at 1 Interdisciplinary Team (IDT) meeting per week
- Four weekends per year: Friday 7 p.m. ÄìMonday 7 a.m. Key Responsibilities
- Participate in admission decisions for hospice at home or inpatient settings
- Develop individualized care plans as a member of the Interdisciplinary Team
- Certify and re-certify patients eligible for hospice care under the Medicare Hospice Benefit
- Consult with attending physicians regarding pain and symptom management
- Coordinate care with attending physicians to ensure continuity
- Develop and monitor the medical component of the hospice program
- Participate in hospice service and policy development
- Support staff and volunteer training initiatives
- Conduct home visits (in person and virtual) as appropriate
- Serve as a liaison and advocate for hospice with national, state, and local physicians, healthcare professionals, partners, and the community
- Perform related administrative functions as required Qualifications
- MD or DO, Board-Certified/Board-Eligible in Hospice and Palliative Care or relevant specialty
- Eligible for licensure in Connecticut
